Medication Form

Medication:

        

 If your child needs medicine at schools, he/she must have a form signed by the parent on file in the First Aid room.  If it is a prescription medication the form must be signed by a doctor.

Change of Clothes:

All children need to have a change of clothes in a ziploc to keep in their backpack in case of accidents, falls, or spills.

Battling Germs:

 

The best way to battle germs is hand washing! Washing for 20 seconds (the length of one verse of "Old McDonald Had a Farm") before rinsing is best.

Fevers:

If your child has a fever, please do not send him/her to school. He/She will spread germs and more children will become sick.

Sickness:

If your child has been vomiting or had diarrhea, he/she should not come to school until he/she has not been sick for at least 12 hours or he/she could spread contagious germs.
