Enrollment Information
The following items are required for enrollment in Arab City Schools:
Picture ID of the parent/legal guardian enrolling the student.
Proof of residence inside the Arab city limits (signed lease, deed, mortgage, most recent utility bill addressed to the parent/guardian at the address being established as the residence, or the order to turn on utilities)
Original State of Alabama Certificate of Immunization (blue card). Please ensure this document has not expired.
Copy of the student’s Birth Certificate (optional)
Copy of the student’s signed Social Security Card (optional)
Complete address of the previous school attended
School records including latest report card, official withdrawal form
Special education records or 504 paperwork, if applicable
Legal documentation establishing custody or guardianship, if applicable
